Hi Madison- We do not have a form that the team member being shadowed signs but that is a great idea. Managers assign and confirm what team members will host our job shadow students. The managers review the expectations of the program, instruct on how to work with the students, etc. but we do not have an physical documents. 

We host a job shadow program for high school students and undergraduate college students. They complete a standard job shadow packet. We also onboard/provide a hospital orientation for students shadowing under the Medical Mentorship program (PA, MD). Let me know if you are interested in the reviewing the documents we use.
